Welcome to Coulee K9 Services! This agreement outlines the expectations and policies to ensure a safe, productive, and enjoyable training experience for everyone involved.

Terms and Conditions:

1. Participation Requirements:

• I confirm that my dog is up-to-date on all vaccinations as required by Coulee K9 Services.

• I confirm that my dog is free of any contagious conditions, including but not limited to kennel cough and fleas.

• I understand that if my dogs shows signs of illness, aggression, or extreme anxiety, they may be asked to leave the class.

2. Safety and Responsibility:

• I agree to keep my dog on a leash or in a designated area unless otherwise instructed by the trainer.

• I will follow all instructions provided by Coulee K9 Services' trainers and staff to ensure my dog's safety and the safety of others.

• I agree to clean up after my dog and dispose of waste in designated receptacles.

3. Equipment and Treats

• I will bring any equipment  requested by the trainer, such as leashes, collars, or harnesses.

• I understand that treats are often used in training. I agree to bring appropriate treats as recommended or use those provided by Coulee K9 Services.

4. Class Etiquette:

• I understand that punctuality is important and agree to  arrive on time for each class.

• I will refrain from using my phone or other devices during training sessions unless permitted by the trainer.

• I agree to respect other participants and their dogs by not interfering with their training or handling.

5. Cancellation Policy:

• I acknowledge that I have received and agree to the cancellation policy as outlined by Coulee K9 Services. I understand that fees may apply if I cancel within the specified timeframe or fail to attend a scheduled class.

6. Assumption of Risk and Release:

• I understand that training sessions may involve inherent risks, and I assume full responsibility for any injuries or damages that may occur to myself, my dog, or my property.

• I agree to release Coulee K9 Services from any liability arising from participation in the training program, except where caused by gross negligence.

Digital consent: By checking the box below and submitting this form, I acknowledge that I have read, understand, and agree to the terms and conditions outlined in this Student Agreement Form. I acknowledge that checking the box serves as a digital signature.
